I'm without a computer at the moment, but I'll look into it.
I've uploaded an epub. Unfortunately, because of limitations of the format, marginal notes seem to have been stripped; also, automatic conversion from latex has screwed up some of the other formatting. I highly recommend reading the PDF version if you can make your device render it.
